Steps to Complete Your DE-964

For Learner License Only Form (DPS/Insurance Copy)

After your student completes the first six hours (Module 1), complete the bottom form of the DE-964 labeled “For Learner License Only.”

Cut along the dotted line and take the “For Learner License Only” form to the DPS with your student, along with the other DPS required items (i.e., Learner License Application checklist which you can find in the Texas Department of Licensing and Regulation Parent Taught Driver Education Program Guide).

  1. Check the box for either Concurrent Program (Module 1) or Block Program (Module 1-12) to indicate your chosen method of instruction.
  2. Make sure your student’s information is correct. (i.e., last name, first name, middle initial, date of birth, gender).
  3. Check the box for Must take Class-Road Rules and Class C-Signs examinations with the Department of Public Safety.
  4. Sign parent’s name on the line for Signature of PTDE Instructor.
  5. List parent’s driver’s license number on the line for Instructor DL#.
  6. Enter the current date for Date Issued.

The control number is unique and assigned specifically to your student. Do not alter. The cost of a replacement DE-964 is $10.00.

For Driver License Only Form (DPS/Insurance Copy)

After your student completes the entire course requirements, complete the top form of the DE-964 labeled “For Driver License Only.”

Take the “For Driver License Only” section to the DPS with your student; along with the other DPS required items (i.e., Provisional License Application checklist which you can find in the Texas Department of Licensing and Regulation Parent Taught Driver Education Program Guide) to apply for a Driver’s License and to take the final road test.

  1. Check the box for Laboratory (In-Car)
  2. Make sure your student’s information is correct. (i.e., last name, first name, middle initial, date of birth, gender).
  3. Enter your student’s Learner License #.
  4. Enter the dates for your student’s Classroom Completion.
  5. Enter the dates for your student’s Laboratory (In-Car) Completion.
  6. Check the box for 30 hours behind-the-wheel instruction, including at least 10 hours of instruction that took place at night.
  7. Sign the parent’s name.
  8. Enter the parent’s driver’s license number and date of completion of the additional 30 hours of instruction.

The control number is unique and assigned specifically to your student. Do not alter. The cost of a replacement DE-964 is $10.00.
